Lucknow: In what is being described as UP’s first such case, doctors at Sanjay Gandhi Postgraduate Institute of Medical Sciences (SGPGIMS) have performed a caesarean section followed by a twin valve replacement procedure on a 26-year-old woman with advanced rheumatic heart disease.An SGPGI spokesperson said on Thursday that the procedure, performed on Jul 21, was also the country’s fourth such reported case.The woman was referred to SGPGIMS at 32 weeks of pregnancy. Evaluation revealed severe calcific aortic stenosis, severe mitral regurgitation and moderate pulmonary hypertension.Doctors said continuing the pregnancy or attempting normal labour posed a serious risk to both mother and foetus.After discussions among specialists from maternal and reproductive health, cardiovascular and thoracic surgery, cardiology, cardiac anaesthesiology and neonatology, the team opted for the combined procedure.A baby girl weighing 2.1 kg was first delivered by C-section. The cardiac team then performed open-heart surgery under cardiopulmonary bypass to replace the aortic and mitral valves.The team comprised Dr M Munshi, Dr Shantanu Pande, Dr SK Agarwal, Dr Saurabh Tripathi and Dr Vivek (CVTS); Dr Neeta Singh, Dr Monali V Khergade, Dr Mandakini Pradhan and Dr Sahithi (maternal and reproductive health); Dr Amit Rastogi, Dr Laraib Shaikh, Dr Shraddha Gangele, Dr Sarbani Ghosh and Dr Shivendra Pratap Singh (anaesthesiology); and Dr Satyendra Tiwari, Dr Roopali Khanna and Dr Harshit Khare (cardiology).SGPGIMS director Prof RK Dhiman congratulated the multidisciplinary team, saying the outcome demonstrated the institute’s ability to provide advanced, patient-centred care through coordinated specialist intervention.
